Dental PPO List
 DeltaWe are Premier Providers only. We are not Delta PPO participating providers, however, as Premier providers we are contracted to accept the fee schedule. When a PPO member sees a Premier provider, we honor the Delta fee schedule, however, there maybe more OOP cost to patient than if they see a PPO provider. We are closed to new ODS/Moda OHP members, however we will continue to serve our current OHP patients. MAC PPO is a newer plan, premier dentists balance bill the difference between the PPO fee and our Premier fee. EPO has NO reimburse for out of network providers, which we are.  RegenceRegence is our area's local Blue Cross. We are participating providers for: Regence MedAdvantage, Lifemap, Regence Oregon, Regence Utah, Regence Idaho, Regence Washington, Bridgespan and HMA.  PacificSourceEffective 6/13/17, we are contracted with the commercial network (Advantage Dental Plan), not the OHP (Advantage Dental) plan. We do not accept the "Essentials" plan which is an HMO.  GuardianWe participate with the DentalGuard Preferred Plus Program only. We are technically not PPO providers.  MedicareWe are currently opted in but non participating Part B providers and NON-participating Medicare DME providers.  United ConcordiaWe are contracted with the Elite Plus Plan (PPO providers). We are not participating in the FedVip, advantage plus or tricare plans. We are not DHMO providers.  Moda MedicalWe are contracted with medical for certain networks including: Connexus, Synergy, Beacon, OHSU PPO, and CCN network. |
